A warm welcome

 

From 1 April 2017 there are changes to urgent care services in the South Tees area, South Tees Access and Response (STAR) scheme has been set up by local practices to improve access to GP services outside of core hours for patients needing urgent care or advice. The STAR   extends evening and weekend opening times for patients with urgent problems from 6 30 pm – 9 30 pm Monday to Friday and 8 am – 8 pm on weekends , patients wanting to see a GP urgently during these times should ring 111.

From 1 April 2017 four GP centres will extend opening times for patients from 6.00 pm to 9.30 pm Monday to Friday and 8.00 am to 8.00 pm on weekends and bank holidays

The STAR (South Tees Access and Response service) service is open to all patients registered with GP practices across Middlesbrough, Redcar and East Cleveland. If you need to speak to a GP on a bank holiday, over the weekend or between the hours of 6pm to 8am Monday to Friday please call NHS 111 (Dial  111) to access the local GP extended hours service

Registered patients will continue to access the practice during normal surgery opening hours. If you need to see a GP outside normal surgery hours which will be 8  am – 6 pm you can still phone Resolution Health Centre – you will be transferred or directed to the GP out-of-hours service. If you are calling after  6 pm for medical advice please call NHS 111 and you will be directed to the most appropriate service to meet your need. This could include a phamacy, out of hours GP service or minor injuries

SMS messaging

As a registered patient at this practice we would like to communicate with you by SMS text message to your mobile telephone. This would allow us to send you appointment reminders and let you know about important things that are happening in the surgery. We will never send patient identifiable information via SMS text message.

Before we can do this we need your consent. If you are happy to receive communication in this way then please ask a member of our reception team to update your record.

It is very important that you tell us about any changes to your mobile telephone number or address.

SystmOnline access

You can also sign up for Systm Online which allows you to book appointments, request repeat prescriptions ,access your medical records and amend any of your details online via the  website. To do this please call into the practice and we’ll get you set up with an online account. As part of the Patient Online initiative you can also now access GP health records , please contact us to arrange access.

We  are improving how we communicate with our patients , so please let us know when you attend the practice if you need information in a different format or any other communication support , we are always happy to help.

We are have now introduced the Family and Friends Test  , this is a way of telling us about the care that you received , your comments are really important and will help to improve the service for you and others – this can be accesed via the iwantgreatcare website if you would like to provide feedback. Copies of the feedback forms are also available from our reception desk.

Resolution Health Centre is managed by the Care Integrated Partnership (CIP) which is a unique partnership between South Tees Hospitals NHS Foundation Trust (STHFT) and Langbaurgh Social Enterprise (LSE). All staff (including GPs) are employees of STHFT and details of primary medical care services in the area can be received from this body.
